Bug - Mountain Ascent - No way off mountain

How often does the bug occur? Every time (100%)
Steps: How can we find the bug ourselves? I'm at one of the last missions which I guess disables fast travel. I'm stuck where you fight the two Bilemaws in the river bed area next to the locked gate. I jumped into the area but there is no way to get out. There is a rock covering the waterfall preventing me from getting to the other side.
What happens when the bug occurs? I'm just stuck in the area w/out a way to get out.
What do you expect to see? fast travel.
