Chamber of clarity not completed

100% completed on chambers of clarity (collected the Fellowship perk) but the screen did not lit up.
Tried pretty much all possible logical way to solve it and things I can do as a player.
Specific Bug Details:
The "Hole" Entrance Glitch: There is an invisible object completely blocking the entrance of the hole on the 2nd floor (the one you have to Force Push/Pull to access the right side).
Phantom Floor Collision: Even the floor area directly under that hole entrance has an invisible barrier, making it impossible to navigate that section of the chamber correctly.
Inconsistent Block Behavior (the block from the 2nd floor ‘hole entrance’) The stone block that is supposed to be pushed down behaves erratically. Sometimes it can be pushed back up, other times it just "smashes down," and frequently it gets stuck vibrating mid-air, creating a permanent collision wall.
not sure if these bug have to do with anything.
