Your latest DXDIAG report suggests your on-board graphics is enabled. You'll want to disable that (in BIOS).
I also suggest you just use 1 monitor while troubleshooting.

Thanks for the additional suggestions. Unfortunately, neither of them resolved the issue.
Would like to note that the in-game lighting is super strange looking after disabling onboard graphics.

Bummer that didn't fix it. If you've confirmed on-board graphics is still disabled in BIOS, you can try:
- reinstalling your NVIDIA drivers while using the "clean installation" option
- disabling overlays, including the ones in GeForce, EA app, etc.
- removing GeForce completely
