Keeps freezing/crashing when I kill and npc or meditate

Whenever I go to meditate or any npc dies, my game freezes permanently and I have to execute the game from task manager and restart it, just for the same thing to keep happening. I've tried reinstalling, repairing, updated all my drivers, but still, nothing works.
