Perk Slot Upgrade not rewarded

Summarize your bug As others have mentioned on here and gamefaqs.com there is a bug prohibiting me and other players where perk slot upgrades are not rewarding upon claiming them. I'm stuck on 9/10 despite claiming every source, I went back to each location and Zee to verify the issue. I'm unable to get achievement requiring you to equip 10 slots worth of perks. Would appreciate a fix please.
How often does the bug occur? Rarely (0% - 9%)
Steps: How can we find the bug ourselves? By claiming the perk slot upgrade I'd imagine it has something to do with initiating another mechanic whilst claiming the upgrade such as hitting the menu button or other actions.
What happens when the bug occurs? I don't get the perk slot upgrade.
What do you expect to see? No additional perk slot upgrade.

I've had similar issues with stuff breaking like claiming a chest and performing and action while the animation is showing and the game crashes.
