Companions being dismissed

Bug: Companions are now being dismissed much of the time when I travel from place to place. Specifically, going into strongholds, going from strongholds to Nar Shaddaa, Mek-Sha and other planets. 

Companions are also still dying when one QTs to the Slugfall Cantina stop on Mek-Sha. Every time one loads in there, there's a pile of dead companions on the ground.
