Ok so I've been dealing with this issue for a long time. I've upgraded my PC, upgraded my internet service and upgraded from a Cat5e to a Cat7 ethernet cable all in the name of frustration and everytime I call into support I get the same run around, I honestly shouldn't weigh 275lbs by now. So I'm very happy to have found this post but I'm also hoping that people still pay attention to it.
Here's my issue. While playing SWTOR on the Jedi Covenant server I have what I feel is a really good ping rate at 21 ms. Over the course of playing what happens is the "signal strength" goes from showing four bars to a red dot. Sometimes it comes back to four bars but most often it shows the red X of death. On most days it just takes me right back to the server select screen where I just click right back into the game but others I get the Unable to load server list. I'm really hoping that someone can help me. So here goes. I'll start with my comp:
If you need any other info from the DxDiag just let me know.

That's more easily seen here since the 7th hop times out.
I'm also attaching a PingPlotter file (mainly due to character restrictions). I've left it run for I'd say a good 20 minutes and that 7th hop reads at 614 ms ping time. Weird part is that when playing Swtor I'm consistently at 21 ms ping time. Also when using Open Visual Traceroute it shows the 2nd through the 7th hop being somewhere in the midwest around Missouri or Kansas before the 8th coming back to Pittsburgh (my location) then the 9th out to California and the 10th stopping in Redwood. That screenshot has been attached as well.
Have you tried a wired connection direct to the modem to see if this persists first up ?
Are you going through another device with your ethernet lead as the results are more expected timings for wireless rather then wired
There is nothing overly out of place in those results apart from higher timings at the modem if this is direct wired connected

Sorry, thought I mentioned it. It is a wired connection but it's through a wireless gateway from my ISP. I know, but i did order a modem (Netgear CM500) and a router (D-Link DIR-655) in hopes that switching from the ISP rental equipment (which is their best model, Cisco DPC3941T) might solve the issue. Hopefully that helps with any further troubleshooting. If you'd like to wait until I have a few days with my new equipment I can post the results next week with new PathPings and TraceRoutes.
Cheers
