Can i just clarify why windows is using 60Hz as default , is that something you are doing intentionally ?
To look at basics first, The DVI cable you are using which one does it match in the image below for pin layout ?

Yes i use 60hz by default. Becouse i dont need 144/120/100 and i cant play all games with stable 120+ fps. So i dont neet it at all.
Not sure what exactly cable i using. But i can choose in windows any refreshrate and its works fine. I am just prefer 60hz.
Cable was with monitor when i bought it.
One more time - i played A LOT of other games with this monitor and system. Like 200++++ And NO such problems at all. Dont search problem from my side.
