That doesn't make sense it matches the dxdiag parameters for the monitor, where as prior it doesnt
Have you updated the video driver as well to the latest version just in case - Link to Driver
Can you paste the contents of the client_setting file back here again as it is at present
My Video Driver is Up to date. and here is the Client Settings

That should be right , it was definitely wrong before , maybe it still is and this isn't overriding things
Are you able to plug that monitor into the video card any other way besides HDMI , I.E DVI or Display Port ?
Even test a different monitor if you can and see if the game behaves
Swtor for some odd reason (one of few gams that does) reads the monitor edid and it must be wrong when it is setting a 120Hz refresh by default.

I attempted to run it a few time while plugged into a different monitor and it still crashed.

When i get a chance will go everything again , but it shouldn't be a setting issue anymore according to the ini file

Definitely a tough one. I think we've exhausted what troubleshooting we can provide with the information you've given us so far. We'll probably need to move on to investigating an MSInfo report from you, but that's a privacy level above what we can handle publicly.
Can you send an email to support@swtor.com with a description of the issue, a list of troubleshooting steps (you can link to this thread), run the LauncherDiag.exe and attach the TGZ files for the guys to investigate?
