"A" deals with the % chance of triggering. If it's 100% (and some are) then you clear this check every time. Otherwise, whatever your percent chance is, that is whatever your percent chance is of triggering.
"B" deals with the dance between potency and tenacity. So far, no one knows exactly how that one works.
But, that's it. It triggers or not, and then it is applied or not. Dodge, foresight, etc -- they deal with damage, not the application of effects. And so it is.

I like your explanation however I will point out that stun through Foresight was "fixed" so you could not do that anymore, it was possible before. As stun is a negative effect, I would think negative effects should not be applied thru Foresight
Yea. Think you must be right. I remain relatively sure that a condition can apply even if a miss (i.e., a dodge), but I'm pretty shaky whether a condition can apply through foresight. In fact, don't recall ever seeing it and I'll happily take no for an answer. :-)
