Any code check-in performed by a developer goes through a process called code review that is performed by peers. At some point, a new build is created that contains these code changes. A series of unit tests are ran as part of that process. If the build passes, it gets handed off to QA. QA then tests these new code changes and possibly performs regression testing around the changes as well. There's a whole system of checks and balances to prevent bugs like this from hitting users, not reliant on one single person.
