In PVE if you used a Clone squad with Cody, Fives and Echo, along with Leia and FOST, and borrowed Phasma as second leader,you could theoretically have Cody do the 212th, with Clones doing 3, they call Leia to make 6, this procs FOST for 7, and he calls Leia again (this can happen), for 10.
If the enemy has speed down, I think Fives would do his double attack, I'm not positive this works with 212th but that would be 11. If Echo is able to assist, that would be 12, I'm guessing that the Fives basic wouldn't proc as part of the 212th, but that would be insane to see.
Fives' basic does proceed as a part of 212th attack. So theoretically, it is possible to have 12 attacks at once.
