Order of operations is:
Remove buffs
Old Ben's Taunt immediately reapplies
Fracture applies preventing any other buffs.
Can't remember where the damage fits into that, but it's working as intended in the case of Old Ben.
When you are cleansing it with Nihilus or Sion it's probably already reapplied from a previous taunt. Also, the initial taunt from Bastila lead doesn't trigger Old Ben's zeta, so the taunt at the start of the battle won't reapply.
