I'm afraid we cannot assist with or review any account actions here on the forums. Please do not provide account details here as these are public boards visible to everyone.
I see you've mentioned using the form already, and just to be sure that should be the ban appeal form outlined on this page. That will be the only way to have this reviewed. If you've done this multiple times and the ban has not been removed, there would be no other option I'm afraid. You'd be able to log back in if the ban were to be removed, but as the form notes, there's no notification provided otherwise.
