EA support have zero idea about this game. This hasn't been the way to deal with bans for years.

However, if you did cheat, there's not much point, they know. Even if it felt like you got banned a long time after you cheated, they do bans in batches. So, they could have detected cheating a long time ago, but only just got around to doing the bans.
In short if you have legit not cheated, fill out the form. If you have cheated, you're just wasting your time. They categorically know.
