Steps: How can we find the bug ourselves? I believe this is specific to my sim
What happens when the bug occurs? I had Lilith Vatore turn Gunther Munch into a vampire just before they had twins. I watched Gunther go through the process of becoming vampire without actually becoming a vampire. When I loaded their save again, Gunther still had Sim needs and nothing to indicate he was a vampire, but he and Lilith have a Master/Offspring relationship and I got a pop up to say he was now a Minor Vampire despite me trying to use Lilith to turn him again at the time.
What do you expect to see? For my sim to become a vampire after turned.

You can see that he and Lilith have a Master/Offspring relationship here. I took out all my Mods and Custom Content to test on another Sim and the Gunther Munch shipped with Get Together in another save, and both changed into a vampire with no issues - dark forms were the same as normal forms, as in their face, skintone, etc. were all still the same. When I use mods to turn Gunther into a vampire, he looks the same in his dark form as he did previously, but the entire facial structure of his human form changes, and he popped out Asian on my last try. I kept trying to equip the Vampire trait through cheats only for it not to work. I tried removing it to then try and add it again, but that didn't work, either.
I originally thought it was because the EA App is telling me I don't have Vampires ( which I have sent an error report on and I have proof I do ), but since the other Sim changed with no issues, I think it's specific to this Gunther.
Have you taken Gunther into CAS to see if he has both forms there? If he does, what happens when you try to edit either form either linked or unlinked?

Yes, I took him into CAS as normal and, despite having been turned and going through the process, he only had one form. I then used a mod to change him into a vampire and took him into CAS that way. Nothing happens when I link and unlink his forms, or even try to edit them. Nothing changes in either form. I also noticed he was wearing a High School Years t-shirt in his dark form, which is not wearable unless unlocked through the ThriftTea shop.
Something for you to try. Create a new user folder. Just rename you existing "The Sims 4" folder to something else or move it to your desktop. Start the game so it creates a new one. Make the sim and place them then save and quit. Copy just the save folder from the previous user folder to the new one then start the game. Do not copy your Mods folder. Play Gunther a little bit and attempt to make him a vampire again through play. Don't use any cheats.
If it works and he becomes a vampire, then it's probably something in your mods folder that caused it which you have to hunt down and either update or remove. Once you find whatever it is, try again with the original save to see if it works and Gunther becomes a vampire. My guess is that you have a CAS CC item that you used on Gunther that is not tagged for use by vampires or broken but that is just a guess. It could be anything - custom skin tone, tattoo, clothes, jewelry, pretty much anything. You'll need to check everything.
