Some of those ideas would probably violate the T for Teen rating, so they're unlikely to be implemented.
If you're playing on PC, there are lots of mods that add more realistic pregnancy and relationship aspects. I haven't used it, but I recommend checking Lumpinou's RPO, as Lumpinou is a talented and reliable modder. There are quite a few around for mental health as well.
I know that's not a satisfying answer, but the sims is a pretty light-hearted franchise.
