I agree with you so I used the sims 4 studio and found the reason it isnt for everyone is the scar sits below the male underwear 

using female underwear does this ( simtimates collection kit underwear)

I have found trans gender female sims going to male do get this aswell. But I agree, But I also do see why the sims team didnt do it

@gooochyeee The scar might not be as visible as with feminine underwear, but it still shows to some extent in your first picture
