I have unlocked the "chef's hat" and it appears the same way, blank space and the game doesn't even allow me to unlock it again, consider that I already acquired it. This post is from July 2019 but I'm having the same problem in 2021, will they adjust?
for me it's the other way around, a third chef randomly showed up and worked for me today, but he wasn't registered as my employee anywhere.... dine out is kinda glitchy i feel like
