I just purchased the Dine Out pack and literally ran into this same problem. I have not figured it out yet but I am definitely about to try and diagnose it. I ordered twice to be sure it wasn’t because my guy got up from the table. They just paid for two dinners to have an unsuccessful date. lol
@dolamae84 In my experience, the main goal of any date in TS4 is to interact with your date 10 times and completing the main goal is all that's needed to have a successful date. Concentrate on your date, instead of the food delivery, and there's no reason you can't still make it a success no matter how bad the restaurant service is.
Note: I don't have Lovestruck. The advice I gave above might not apply if you do have Lovestruck.
