Hello.
I am having the same problem.
I am playing Sims4 on PC, Japanese and suddenly one day the alt key stopped working.
List of things I did to fix the problem. ↓
・Remove mods.
・Play with new save data.
・Initialize Sims4.
・Fixed the game on EA app and STEAM.
・Reinstall Sims4.
・Turn off the overlay in EA Apps and STEAM.
・Update windows.
・Intel update.
・ Keyboard driver updates.
・Disk cleanup and defragmentation.
・Clean boot.
・Change keyboard settings.
・New keyboard.
And much more.
Still, it did not fix the problem.
I now use the screen keyboard only when I want free placement of objects as a temporary fix.
It is a bit silly, but the ALT key on the screen keyboard is reflected in the Sims4 game.
I hope one day this problem will be fixed or I will find the cause.

Terminate the program you have running that uses CTRL+SHIFT+C (probably a screen capture program) before starting the game.

If the suggestions in #22 and reading “Common Pitfalls” on this page (https://answers.ea.com/t5/Game-Issues/FAQ-Common-Issues-with-Cheats/td-p/13573997) don't solve your problem, then I I'm in the same trouble as you are.
Using the “On-Screen Keyboard” was helpful for me, although it is not a radical solution.
