That's because they all are now hanging out at cemeteries since the Life&Death patch lol.
Seriously though, I have the same issue like you. Only few sims spawn and most always the same ones. Only when I visit my cemeteries, an invasion of sims spawn there from most every pack I own. It's ridiculous.

I haven't thought to visit everywhere on my game and check if they're all mysteriously in one place, but I'm definitely going to try that now because you never know, could be the same thing happening on my save so thank you for giving me the idea 😃
It's true. In my game there are a lot of Sims at cemeteries whenever I visit. But when you go to a bar or elsewhere there ain't hardy any. The worlds are almost dead. Next time I visit a cemetery I will make a screenshot and post it here.

So there are mostly 12 and more sims spawning at cemeteries in a time period of 10 real time minutes. Right now I counted around 10-12 in 3 different cemeteries in 3 different worlds.
(My Cemetery in Strangerville) 10 sims on one side and across are 2 more sitting at the bench.
(My Cemetery in Chestnut-Ridge) Here are 13 sims including my sim.
That's the most alive place I've seen anywhere compared to my game (pun totally intended 😉) I don't own life and death so have been visiting every bar, club, pub, swimming pool, community lot you name it and even in Windenberg there was absolutely no one else at the gym or the swimming pool it was just my sim and that world is usually the most populated on my game even compared to San Myshuno! Until an answer is found I've turned on all the options in neighborhood stories (apart from adopt a pet) for both played and unplayed households and I'm going to see what happens. I do wonder if it is a result of the L&D update that we got even if we don't own that pack??!!
