Sims can't have bladder failure. Turned off autonomy and left my sim in water to complete formative moment to have a bladder failure in the water and it doesn't happen nor in pool, nor in open water. Was walking towards the nearest bathroom, which was pretty far, still no bladder failure.

could you try moving the family to another lot and back to see if that changes anything ? Cheat any money you need.

The bladder need stays red at the lowest point, it does not move.
Is it just one Sim that has this issue ? Did you possibly freeze the need somehow ?
Did not check on other sims in the household, they have the steel bladder trait, I only noticed it on my child sim who needed to complete a formative moment. The needs for the child sim are not frozen, needs are enabled, no steel bladder trait.
