scry lost souls give soul although none could be saved

I clicked "Me too" but my problem is actually different. I can't seem to unclick "Me too". Sorry!

When I choose "Scry for Lost Souls" and it prompts whether to try to save the lost souls, the game always says that none could be saved but one soul is added to my inventory. Every time I "Scry for Lost Souls" I got one soul. but then one is added to my inventory.

No mods or cc downloaded.
