Sim died, no grim reaper, no grave

I am playing on a legacy family (no mods or CC in my save) and one of my sims died but the grim reaper never appeared nor did a grave get left behind. I got the notification in the top right that they were freezing to death (on the home lot), but as soon as I clicked it off and before I could do anything about it, I got the notification that they had died. The grim reaper never showed up, but then I got the choice whether to add the ghost to my family or not. I chose not to do that (before I realised no grave had spawned) and now I have no way of bringing them back. I checked all the other sims inventories and the family inventory and it’s not anywhere. I even got the notification since that their ghost is fading away, but they haven’t appeared as a ghost on the lot once and grave/urn never spawned. I tried getting an urn from debug to see if I could assign it once the ghost appeared but it never did. Any ideas ??

    What you described is normal for sims who are not in the active household and die out of sight.  And if you got the fading away wall post, the sim has probably been culled and gone forever.  But maybe not.  If the sim is known to an active sim and still in the relationship panel, you can save them.  Life & Death gave us the ability to call a ghost and ask them on a date.  If you have Life and Death or if that option was added to base game, then you can use that to get the ghost on the lot where you can ask them to join the household, so they become playable and can be saved. Once the ghost is a playable sim, then you can keep them around for however long as you like.  To get an urn for them though, you'll have to resurrect them and then have them die again while a played sim. You cannot assign any of the urns in debug to a sim.  The sim has to die and create an urn for that.
