Sim trying to pick up non-existent diaper.
My sim keeps getting the throw away interaction for a diaper then walks outside and gets the uncomfortable from unpleasant surroundings moodlet then the interaction gets cancele...

sounds like the diaper somehow spawned to a inaccessible place.
Are you using mods/cc ? Would you be willing to try a mod workaround ?
You either need MCCC or TwistedMexi's AllCheats.
With this, you can check which objects are on the lot and remove them.
Once you put the mod in, open the cheat console and type:
- objects.list iaper (it's correct that the D is missng)
- If you get results there should be an object ID next to it so type: objects.destroy <ID>
If you don't get any results, it could be named differently so try objects.list to get all the objects on the lot to see if if it's among them.
Another option is to use S4Studio to find it.
If you are on console, neither will work though.
