in the doctor career, NPCs are mainly set decoration. There are a few interactions you can do with them and other doctors do play a role in the lower levels and orderlies do make beds once you advance (iirc)

Your Sim basically needs to do everything. So you need to click on the Sims and admit them and then go from there. It's a bit frustrating as you will always have a line but that's how it works.
