Most of my sims that I don't play while I'm playing others (I'm a rotational player) seem to get fat if I'm not playing them unless they have the "Active" trait then they will somewhat retain most of their muscle. At least in my game most of my non played sims seem to enjoy eating more than working out! Especially since I have a diner in the world now!
The way I understand the OP is that exercising in the game, that is part of the gameplay and so can be done for different reasons and gain; also has the effect of drastically, and quickly, changing the looks of the sims, not always desired by the player, or understood to be the result.
