Vengeful Deity Achievement Not Updating

Hi, I have been trying to complete the "Vengeful Deity" Achievement, in which players need to acquire the ten unnatural causes of death for the Sims. I am aware they are: 1) Starvation; 2) Fire; 3) Drowning; 4) Electrocution; 5) Cardiac Explosion; 6) Hysteria; 7) Mortification; 8) Overexertion; 9) Cow Plant; 10) Meteor. From my time playing this game, I have gotten the following done (Don't Mind the names, I just wrote whatever came to mind): 

1. Meteor (Sim Name: Alyx Lee via Telescope) [Counted as 1/10]
2. Drowning (Sim Name: Jaundice Kim via Pool[Did Not Count Even Without
   Changing Sims; Stayed 1/10])
3. Drowning (Sim Name: Ready To Eat via Pool [Counted as 2/10])
4. Mortification (Sim Name: Bleu Drake via Embarrassment from Peeing Pants, Embarrassing Convo, Seeing Something They Didn't want to see, and Privacy Invasion [Did Not Count Even Without Changing Sims; Stayed 2/10])
5. Starving (Sim Name: Peter Drake via Starvation [Counted as 3/10])
6. Fire (Sim Name: Ho Bo via Cooking [Did Not Count Even Without Changing Sims; Stayed 3/10])
7. Cowplant (Sim Name: ? via Eating Cake [Counted as 4/10])
8. Fire (Sim Name: Chin Ko via Cooking [Did Not Count Even Without Changing Sims; Stayed 4/10])

As demonstrated above, I only have been getting 4/10 for my unnatural deaths, despite getting 6 different types of deaths. I am not sure how to fix this but I would like this to work before I continue collect these. I did get two fire deaths and to no avail a progress count. What am I do wrong and how can I fix this? Here are the pictures of what I have now in response to the most recent death.

Note: I already restarted the game and everything. Also, whenever a Sim dies and doesn't count, I do not save until the next death before determining if the death was counted or not.
