I get this happening with one of my Sims as well. She keeps forgetting her own children. As soon as they move out into different homes it's like they've never existed & she's like, 'who are you again?'. I end up using the relationship cheat on MCC to fix it. I assume it's because of the mods I use to get the Grim Reaper as a fully playable character that makes things go a bit wonky sometimes.
