So I think what is happening, is the outfit was painted on a white skin baby. That outfit is in your crib package. But the maxis baby is the clothing part, with the new outfits you have a holding baby outfit and a baby in crib outfit. I know it sounds a little confusing, but basically you have conflicting happening with an outdated and a new outfit or baby default skintone.

Yes, I'm familiar with how the bassinets and baby clothing work, I'm a CC creator and looked extensively into how to create a functioning bassinet without having to override the default one.
But, the thing is I have NEVER, EVER, not once played with babies. I have never downloaded anything for babies or even had a pregnant sim until tonight so that is what was so confusing to me. I had testers try out my bassinet to see if it would function because I didn't even know how to have a baby.
Wonder why it would conflict when this is a brand new, just created TS4 in my documents from 3 days ago, and I've only been renovating houses, I haven't even played besides creating my family, and they were created with a toddler in CAS, no pregnancy.
