If you're having trouble with it working and you want the shirt (and are on a PC rather than a console so you can do cc) it is super-easy to make the special hidden clothing items (like uniforms, stuff you get at vendors' stalls or other locked items) into normal unlocked CAS items. I do it all the time for outfits I want to use that I'm not going to be doing the game play to get.
You need to install Sims 4 Studio. It's free. Found here: https://sims4studio.com/board/6/download-sims-studio-open-version

I looked around on the "EA Answers" site, trying to find a thread about this problem, because I'm pretty sure I've heard others mention issues with the "buy and wear" option, but I couldn't find one (there might be one, I probably just didn't guess the right search words). You might want to submit one there.
This is the first time I've ever bought a t-shirt from there, so I didn't know what it was supposed to look like. Thanks for the suggestions. I don't use CC (personal preference), but I'll try using buy and going into CAS next time and see if that works.
