I will hold off judgement of the implementation of this until it's released, but I do want to add my voice to how I hope it's done.
I don't want each and every sim in-world forced to have this displayed. I don't understand the need for it to be visually showing in live mode. This is a single player game and there is not a single other person who sees my game but me. It's not like this is a virtual world where you meet other RL players controlling an avatar and you would need to know how to address that particular avatar.
I'm happy the team is adding it for those that needed/wanted it, but for me personally this is just a game. I control the lives of these little computer generated beings and I decide who/what they'll be. I will most likely not ever use this feature so I hope the default setting (as in, if you don't choose any type of pronoun for newly created sims) will be just like it is now.
