I think you can give the vet's wellness treats to animals that aren't in your household. I usually have my pet-owning Sim buy a bunch, and they'll often give a couple to any sick animals they run into.
As for the guilt -- I felt terrible when a child was struck by lightning in TS2. And one of my Sims died of embarrassment after his girlfriend in TS4 turned down his proposal. I try to keep them as happy as possible.
