I have never created a trans sim, but there's one former townie who married one of my male sims, who I changed from cis male to trans male who could get pregnant. Nothing about his life changed - he's just a gay man who happened to give birth to twins. But I consider him trans because my brain likes that better than cis men getting pregnant other than by alien abduction.
Now that I have that one trans sim in the game, I am considering making one of the twins grow up nonbinary. But I play rotationally so it will take a while for him to be old enough.
