Some diseases are easy to diagnose, some are not. If you are not very sure about the disease and do not have any guide at hand, just avoid the task. You do not need to get gold every day to advance. I used Carl's guide when I played my doctor sim.
It has been some time since I last played doctor and there was no issue back then, except that it takes a terribly long time for sims (both doctor and patient) to get around the place.
