I finally got the hang of it by repetition. I have not had a Sim in the active physician career now for quite some time, though. I also used to have my budding physician put the patient through every inspection imaginable. Eyes, ears, nose, throat, temperature, specimen. He was very thorough, which is why, I'm guessing, he ultimately became a World Renown Surgeon, as he was in Sims3. Straight to the top he rose. Nowadays, I use a cheat. I'd much rather follow my Global Superstar as he rises in fame. My bad. But, in a household as large as mine, it's hard to follow every Sim on their path to success. Just keep watching and see which symptoms the patients exhibit and eventually, you'll know what ails them. But, you'll still have to put them through a series of 'tests' and the basic check-up before you can hazard a diagnosis.
