Gonna clear up confusion about The Sims 4 as I see a lpsome of this from this thread.
NPC dying and not having gravestones are something that was an issue in the game years before. However because of the constant feedback back then they changed thin ( Cats and Dogs patch? I can't remember since it was years ago). Basically it is now possible to build Cementeries with NPC.
When a Sim Die it was changed to which if a NPC die off screen, their gravestone or Ur. will appear on their home lot. (Mostly outside). However if the game cannot find a suitable place for the urn/gravestone to place, it will automatically go to the household inventory.
After knowig that patch I remember creating a Graveyard full of NPC Sims. I still do it in m legacy save file. When I visit a house and saw a urn, I automatically place it in my sims inventory and put it on the graveyard.
