I like to give makeovers to NPCs from manage worlds, without ever having played them (ie, i note their name while playing and then go to manage worlds later and edit them). I have noticed that in some cases, NPCs lose their role after their makeover, even though I've never entered live mode with them. Is there any way to determine which ones will retain or lose their roles after being given a makeover?

I should look into that, because one role that definitely fits there is Paparazzi. To quote the post there:
This is an extremely fragile career. Simply editing the sim in CAS will remove the career, not to mention moving them around and changing the played status also does that. So, in order to successfully get a custom paparazzi, you'll need to do the following:
Evict the paparazzi sim.
Have a surefire way to get the sim you want to be a paparazzi on the same lot (clubs can help).
Get their SimID with this cheat:
sims.get_sim_id_by_name "[Sim's First Name]" "[Sim's Last Name]"
Equip the career using the career cheat with SimID.Remember, playing with the sim again will remove their role.
Plus moving some butlers around caused them to lose their butler status.
Unfortunately, exactly when these careers get removed I don't know where to look. However, should this happen, the only thing I can suggest here is moving them in, playing with them, and following this guide.

I've actually successfully given makeovers to game-generated paparazzi before and had them function as paparazzi in my other saves, when I place them/evict them/never play them.
I might have to do my own experimenting too at some point. I find that what works best is to make them over via Manage Worlds, not via cheats and to make them over in live mode. But still, it is only certain types of NPCs, not all of them.
