I'm not in my game at the moment, but one way to find out if this is a problem with your current game save or not. First, if you play with mods, remove those, and delete a file called localthumbcache.package that is in your The Sims 4 folder. If you don't play with mods or CC, then just delete the file.
Then go into your game and see if the issue is still happening. If you don't see the issues, then you'll need to see which mod is outdated. If you don't play with mods and the problem has been corrected, then the issue could have been the localthumbcache file. This file does get corrupted, and from time to time needs to be deleted so the game can make a new file.
If you have CC or mods that would affect your sims, then after you have tested to see, don't save after testing. This will keep things in place as far as CC is concerned.
