I rarely use fireplaces even when they are in the house and I never upgrade them, but one of my sims has two and recently they are always on and have never caught fire. Maybe the fire incidents are related to a sims's traits or skills?
Both Sims have a rather high level of Handiness Skill, so that blows that theory. But thanks for weighing in. :)
