Have you ever had a baby/toddler/child taken away?

I've had children taken away a lot! It seems no matter how hard I try there's children being lost left and right. I don't even mean to so either my sims are horrible parents or I'm not doing something right. Most the time it's from starvation. So far in my currently play through no children have been lost so fingers crossed.
