Would probably want to include screenshots of what you're seeing as well as what is 'actually' being worn by that sim - which can be obtained by loading up the sim in 'Sims 4 Tray Importer'

- if nothing else when you load a sim in that if you see anything in red, there's a problem. But if you see anything you don't recognize, it will also show the file location, and you can figure out what that something is.
