How do fears work?
I havent explored fears much and dont really understand them. What can your sims fear and how do they develop new fears?

This seems to be a rare bug (reported here, has only 4 "me too"s: https://answers.ea.com/t5/Bug-Reports/Extinguish-Self-and-other-Sims-does-not-resolve-the-Fear-Of-Fire/td-p/12612735)
You could try the Fear-B-Gone potion or the reward trait from the Outdoors Enthusiast aspiration (from OR, makes sims immune to fire).
