How to get a 5 Star Vet Clinic? (ANY HELP WOULD BE GREAT)

It seems impossible to reach a 5 star vet clinic. I have worked forever and I now have a 4 star clinic. I do not know how to get to a 5 star one, I don't think I can improve anymore. My clinic is the ...

I cannot get past 4, I get to 4 stars and then it drops to 3.5. At this point, I'm about to simply be happy with 4 stars.
