How would you make sims more challenging?

Let's start with some basic premises for the series, so we know what we have to work with:

  • It isn't fun when your character just drops dead with nothing you could have done to save them. There needs to be some way to avoid the worst. The most popular form is a HP bar to show just how much more your character can take, but that doesn't really mesh well with the overall sims style of day-to-day life.
  • Sims will never be a proper esport. The userbase is not known for their split second reflexes and brilliant tactical acumen. Frankly, requiring these sorts of skills would turn away a lot of long time simmers. So while it should be possible to opt into harder challenges, the default should not be too punishing.
  • Traits and moodlets are the primary tools you can use to affect sim durability. Skills are best avoided as a challenge mechanic, since depleting them as a consequence of a challenge feels punishing enough to discourage many players and depleting them in the course of a challenge is just going to upset people. Money and needs can be used, but then you have the problem of creating a meaningful loss for an established sim that doesn't also completely wipe out a lesser one. And when working with moodlets, we all know the weaknesses of that system.

Given all of these, how would you practically add challenge to your sims games? Bad things might happen (burglars are a very popular request), but i don't know how having to buy a replacement item and stack positive moodlets until a negative one wears off is a meaningful setback. So I'm curious what hardship lovers would add to the game to up the challenge without falling into one of the abovementioned traps.
