What I've discovered is that random flirting is based on gender preferences. When I place a new sim in my world, I'll purposely make sure that they don't have a preference for either gender ( I do that through MCCC). I want them to develop as I play them. If all the NPCs have gender preferences generated, they will flirt according to what they're into. And it's very random. Just more drama that EA likes to put into the game, much like roommates.
To add further: a random flirt won't necessarily add anything that you can visually see to the social bar. One simple interaction may not be enough to see. But once a sim has flirted with another, it opens the door to further flirting and it tends to happen a lot after that point.
