If Create a World is possible within the game's base Engine (as apparently they've clearly said Open Worlds doesn't work with the game's engine and can't be made to do so), and if they were to provide us with a Create a World tool, I'd actually expect it to be among the last, if not the last expansions before they switched to TS5... assuming that they anticipated a 6 month or more delay between the end of TS4 and the release of TS5. Or if there is no TS5.
Basically I'd expect it to be a "farewell" expansion pack to the Sims 4 community. "Hey guys, we're not supporting this game with new worlds any more, so here's a tool to make your own for just US$40 to tide you over until TS5 is ready and we start this dance over again."
And I'd expect we wouldn't be able to build large worlds, just neighborhood sized ones like Forgotten Hollow or Glimmerbrook. And no editing existing worlds either.
But realistically, I don't expect to see such a thing at all. I expect that if possible, it may be a feature in TS5, which will roll out within 2-3 months of the last TS4 expansion... whenever that is.
