I'd imagine the entire spine would be sore from walking around pregnant and a much larger chest due to that pregnancy. I also don't see any aid for pregnant women in this game either, like items to put behind a womans back to help alleviate the discomfort with proper support, especially in the lower half of the spine where there is a natural bend inwards.
