I like a lot of what's suggested here, by OP and others. Most of all:
GTW's hospital, if it had been more interactive (like having Sims go there when they're sick) would've been great for pregnancy additions beyond giving birth there: prenatal care, ultrasounds, check ups etc.
But even having the chance of getting pregnant a little more, well, chancey would be nice.
I definitely love the fact that pregnant Sims can wear any clothing. I don't think I owned any specific maternity clothing items when I was expecting and since I wasn't planning on multiple kids, they felt like a waste of money. So not having to be restricted in the game is great.
What I would also love to see is a midwife or doula for home births. Maybe have a Sim that can be "hired" to come visit the pregnant Sim for a time during the pregnancy and do breathing exercises, talk them through worries, then be there for the birth if it's a home one.

I know we have that trait which makes Sims more fertile; but I think it cold be an idea to randomly make Sims more fertile than others? So whilst one Sim gets pregnant after 1 attempt; another takes 15 attempts that kind of variation and a couple could visit the hospital to find out which of them is more/ less fertile.
